Employee Benefits




Health Insurance

As a member of the Southeastern MA Health Group (SMHG), the Town offers Blue Cross Blue Shield of Massachusetts HMO and PPO health insurance plans.  Open enrollment for health insurance in held in May for July 1st coverage. Normally, you can only be added to health insurance  during open enrollment.  However in the case of certain “life events” you can pick up coverage for you and your dependents outside the open enrollment period.  Click here, to read more about Life Events

The Affordable Care Act requires that employers provide a Summary of Coverage and Benefits (SBC) for each of its plans, to all eligible employees. This information is available here on the website, or will be provided in writing upon request.  Below are FY19 Summary of Coverage and Benefits for the Town’s Health Insurance plans.

Pharmacy Coverage

Blue Cross Blue Shield of Massachusetts has chosen Express Scripts as your mail service pharmacy. You can order up to a 90-day supply for most long-term medications, which can save you time. You also can save money because you’ll have a smaller co-payment for most medications when you purchase them through the mail service pharmacy instead of a retail pharmacy.

BCBS Fitness and Weight-Loss Reimbursement

Dental Insurance

The Town will offers one (1) dental plan through Altus Dental Insurance Company.  In addition to the $1,000 annual maximum per participant, the plan includes a Preventive Rewards Program which covers first and second cleanings and exams, X-Rays, Flouride Treatments and Sealants, all outside of the annual maximum.  Click here for FY19 dental rates.  Click here for Altus Enrollment Form

Basic & Voluntary Life Insurance

The Town administers, through Boston Mutual Life Insurance Company, two group life insurance policies to it employees at the time of hire. They are the Basic Life Insurance Plan and the Voluntary Plan. The Basic plan provides a $4,000 Death Benefit and a $4,000 Accidental Death and Dismemberment Benefit. The Town pays 80% of the cost and the employee pays 20%. The Employee Life Option Plus (Permanent Life) Plan provides coverage up to $400,000 and is available at the employee’s expense. Policies exceeding $100,000 require a completed Evidence of Insurability forms:


Vision Care Plan

The Town offers a voluntary vision care plan through Davis Vision. This is a network plan with a very broad network of providers.  To find a provider, click here.

Employee Critical Illness and Accident Insurance Plans

The Town offers Critical Illness, Plus Specified Disease Insurance and Accident Insurance coverage through Boston Mutual Insurance Company. These plans are administered by LifePlus Insurance Agency, Inc. in Marshfield, MA. To enroll or for claims assistance, contact LifePlus at (866) 511-9222.

Short Term & Long Term Disability Insurance

Flexible Spending Accounts (FSA)

The Town contracts with Cafeteria Plan Advisors (CPA) to manage its Flexible Spending Account program. CPA provides FSA services; including a web site and cell phone app to allow participants to manage FSA medical and dependent care dollars online, and a pre-paid FSA card to use when paying FSA eligible items.

FSA’s are a way for employees to set aside pretax dollars to pay for out of pocket medical/dental expenses, prescription drug co-pays and dependent care costs. Mansfield employees have the option of establishing a FSA through CPA. The FSA plan year is October 1st – September 30th. The open enrollment period for this benefit is the month of August/September. Benny cards are provided at no cost to the employee and are meant to be used like a credit card at the time of service instead of paying out of pocket and submitting receipts for reimbursement later on. Employee Assistance Program

The Town offers an employee assistance program called AllOne Health through MIIA (the Town’s health insurance administrator). AllOne Health provides assistance with personal issues affecting job performance and other areas of the employee’s life. Deferred Compensation Option – 457 Plan (available to some employees)

Discount Health Club Membership

Pet Insurance

The Town offers pet insurance through Nationwide Pet Insurance. This insurance can be used at any vet. There are multiple plans available and coverage is available for: accidents; common/serious illnesses; hereditary conditions; procedures/services/x-rays and wellness services. Retiring Employees

IMPORTANT TO NOTE:  If you are planning to retire, we ask that you complete a Retiree information Form sheet and forward it to the Treasurer’s Office so your retirement benefits can be coordinated.  If you are 65 years of age or within a few months of turning 65, you should send this form in at least 3 months prior to retirement.   If you are not nearing age 65, please turn the form in one month prior.  The Retiree Benefits Information explains the benefits for Mansfield retirees.
